CHRONIC DISEASE MANAGEMENT
Our Chronic Disease Management (CDM) Clinic is staffed by highly qualified registered nurses.
CDM or Care Plan system is available to anyone suffering from a chronic disease and who fit the criteria.
We have highly qualified and experienced General Practitioners and nurses who can effectively manage chronic diseases such as heart disease, high blood pressure, asthma, chronic lung disease, diabetes, arthritis and depression.
